The terms of the Agreement which will bind you to the laboratory will be explained to you, and you will be given the documents useful for your activity in connection with the laboratory.
In accordance with decree 2002-660 of April 30, 2002 relating to the conditions for transmitting biological samples to medical biology analysis laboratories, when the sample is taken by a health professional, the prescription must be accompanied by a transmission form. Samples containing all the information relating to the patient and useful for carrying out and interpreting the examinations.
Patient identification: surname, first name, date of birth.
Warning: Birth name compulsory for blood groupings.
Patient's residence: main address and / or temporary address of the patient, telephone
Prescription: if the prescription is not attached, enter the name of the prescriber and the prescription date, as well as the analyzes requested.
Note: This information is requested in the case of a renewable prescription already sent to the laboratory.
Request for analyzes without a prescription: this document specific to each laboratory will be sent to you by your reference laboratory
Refusal of analyzes: this document specific to each laboratory will be sent to you by your reference laboratory
Sampling conditions: date and time of sampling, name of the sampler, fasting patient or not
Treatments, clinical information: Specify the treatment and dosage, time of last intake, weight and height of the patient in case of drug dosage. Specific clinical information sheets may be sent to you by the laboratory
Optional administrative information can also be transmitted such as the social security number, the name of the insured (if different from the beneficiary of the care), the health insurance fund, the mutual insurance company (name, AMC number or prefectural code and Expiration date).
If the administrative information is not filled in, the total amount of the file will be billed to the patient. A sheet of fees for medical biology acts will be given to him after receipt of the payment by the laboratory.
The information provided on the transmission form does not exempt the samples from strict identification.
